The Medical Technology Licensure Exams are fast-approaching! Here are the most important information you need to know to be able to register:
1. IMPORTANT DATES.
Deadline of filing: July 26, 2017
Date of exams: August 26 & 2017, 2017
Venues: Manila, Baguio, Cebu, Davao, Iloilo, Legaspi, Lucena and Tuguegarao
2. LIST OF REQUIREMENTS
- NSO / PSA Birth Certificate
- NSO / PSA Marriage Contract (for married female applicants)
- Transcript of Records with scanned picture and Remarks "For Board Examination Purposes" (First Timer & Repeater)
- Payment: P900.00
*After three (3) unsuccessful examinations, the applicant is required to submit a Certificate of Completion of a REFRESHER Course from a school of medical technology recognized or or accredited by CHED- FTB.
3. STEP-BY-STEP ONLINE REGISTRATION
a. Before you start, you must have the following on hand:
- Your e-mail address (with password)
- Your mobile number (ex. 09271234567)
- Personal data (including date of graduation)
- ID photo in .jpg format saved on your computer, phone or tablet. (You can request for an e-file if you have your ID photo taken at a photo studio.)
- Cedula (residence certificate)
